Last spring a new part-timer joined our shop. During her self-introduction I was answering the phone, and I caught only the beginning of her surname. It sounded like "Hayashi", so from that day I called her Hayashi-san.
はやしさんはいつも「はい」と返事 をした。一年 の間 、私 は一度 もまちがえていると思 わなかった。同僚 の名前 を覚 えるのは得意 だと、自分 では思 っていたのだ。
訳 English
Hayashi-san always answered with a "yes". In a whole year it never once occurred to me that I had it wrong. I had thought of myself as good at remembering colleagues' names.
今年 の春 、はやしさんが大阪 の店 に行 くことになり、送別会 をすることになった。部長 に「字 がきれいだから」と言 われて、私 がカードを書 くことになった。みんなの名前 を一人 ずつ書 いているところカードを書 いているところだった。 I was in the middle of writing the card.だった。

This spring it was decided that Hayashi-san would move to the Osaka shop, and that we would hold a farewell party. The manager told me my handwriting was neat, so writing the card fell to me. I was in the middle of writing everyone's name one by one.

Just then I noticed the writing on the name tag on the desk. It said "Hayashida". My face went hot. In a whole year, I had never once asked her about it. When I checked with the manager in a small voice, he laughed and said, "Everyone knew."

On the day of the farewell party I apologised. Hayashida-san said she had noticed on the third day, but that by then it was hard to say. According to my colleague Tanaka-san, Hayashida-san likes the shorter form and tells her friends the same thing. When she said, "Even after I move to Osaka, please go on calling me Hayashi-san," my embarrassment turned, just a little, into relief. The name I got wrong has become the name she chose.
